The undersigned Member, or their legal parent/guardian (henceforth shall be referred to as Member), acknowledges that Taekwondo is an active sport. The undersigned fully understands that: (A) Taekwondo training involves risks and dangers of serious bodily injury; (B) these risks and dangers may be caused by the Members own actions or inaction’s, the actions and inaction’s of others participating in the training, and/or the conditions in which the training takes place. The Member further releases Baek’s Tae Kwon Do Academy, its staff members and all fellow students from any and all liabilities resulting from personal injury or loss of personal property. The Member also agrees that no other representation is made other than what is agreed to in writing herein. Failure to use the services provided by Baek’s Tae Kwon Do Academy will not relieve the member of payment. The Member also acknowledges that the Member has read and understands the terms and conditions of this Waiver and associated Terms & Conditions.
